Powering the Care Models of Tomorrow: mHealth’s Pivotal Role in Care Coordination and Accountable Care

Tuesday, December 6, 2011, 8:00–9:15 AM

Overview: Mobile tools make it easier than ever for providers perform the role of central care coordinator. At many hospitals across the country, mHealth is playing a critical role in the drive to deliver patient-centered care, including by employing telehealth to speed care for stroke, cardiac and other time-sensitive patients; arming providers with tablets that can access the organization’s EHR; and using smartphones to access patient data and make timely treatment decisions.

Government agencies from HHS to NIH also are implementing a host of initiatives to encourage healthcare providers to use mobile technologies, and to move from a facility-centered model to a patient-centered one. In particular, regulators are revealing how mobile-enabled care coordination will be at the core of Accountable Care Organizations going forward.

This executive breakfast panel discussion, hosted by FierceMobileHealthcare, will showcase the mHealth solutions that providers have implemented to improve care coordination and increase efficiencies across the care continuum. Panelists will include executives from leading provider and government organizations who will discuss mHealth’s pivotal role in accountable care.

Key learning objectives:

  • Discover how telehealth and mobile programs are improving stroke, psych and home care protocols at Inova Health System, Fairfax, Va., enabling multiple clinicians to consult on a single case in real-time.
  • Find out why HHS is pushing hard for healthcare data flow over multiple mobile platforms, and a centralized, device-agnostic approach (and what that should mean for your mobile strategy).
  • Dig into NIH research on mobile-enabled care for cardiac patients, addiction treatment, and other specialty areas that may ultimately prompt CMS to create a payment structure for telehealth visits.
  • Uncover the clear path from mobile-enabled coordination of care at a hospital to the eventual creation of accountable care organizations.
  • Learn about HHS-backed experiments in community-based care coordination using mobile technologies, and how that should inform hospital and health systems' mobile planning.
